കഠിനമായ വേദന ചികിത്സിക്കാൻ ഒപിയോയിഡ് റിസപ്റ്ററുകളിൽ പ്രവർത്തിക്കുന്ന ശക്തി കുറഞ്ഞ ഒപിയോയിഡ് വേദന മരുന്നാണ് അൾട്രാം [3]എന്ന ബ്രാൻഡ് നാമത്തിൽ വിൽക്കുന്ന ട്രമാഡോൾ,[1] വായിലൂടെ എടുക്കുമ്പോൾ, വേദനസംഹാരിയുടെ പ്രവർത്തനം സാധാരണയായി ഒരു മണിക്കൂറിനുള്ളിൽ ആരംഭിക്കും.[1] ഇത് കുത്തിവയ്പ്പിലൂടെയും ലഭ്യമാണ്.[13] ഇത് പാരസെറ്റമോളുമായി (അസെറ്റാമിനോഫെൻ) ചേർന്ന് ലഭ്യമാണ്.

സാധാരണ ഒപിയോയിഡുകളുടെ പോലെ, മലബന്ധം, ചൊറിച്ചിൽ, ഓക്കാനം എന്നിവയാണ് സാധാരണ പാർശ്വഫലങ്ങൾ.[1] ഗുരുതരമായ പാർശ്വഫലങ്ങളിൽ ഭ്രമാത്മകത, അപസ്മാരം, സെറോടോണിൻ സിൻഡ്രോം ഉണ്ടാകാനുള്ള സാധ്യത, ജാഗ്രത കുറയൽ, മയക്കുമരുന്ന് ആസക്തി എന്നിവ ഉൾപ്പെടാം.[1] വൃക്ക അല്ലെങ്കിൽ കരൾ പ്രശ്നങ്ങൾ ഉള്ളവരിൽ ഡോസ് മാറ്റാൻ ശുപാർശ ചെയ്തേക്കാം.[1]ആത്മഹത്യാസാധ്യതയുള്ളവരോ ഗർഭിണികളോ ഇത് ശുപാർശ ചെയ്യുന്നില്ല.[1][13]മുലയൂട്ടുന്ന സ്ത്രീകളിൽ ശുപാർശ ചെയ്യുന്നില്ലെങ്കിലും, ഒരു ഡോസ് എടുക്കുന്നവർ സാധാരണയായി മുലയൂട്ടൽ നിർത്തേണ്ടതില്ല.[14] ട്രമഡോൾ കരളിൽ ഒ-ഡെസ്മെതൈൽട്രാമാഡോൾ (ഡെസ്മെത്രമാഡോൾ) ആയി പരിവർത്തനം ചെയ്യപ്പെടുന്നു. ഇത് μ-ഒപിയോയിഡ് റിസപ്റ്ററുമായി ശക്തമായ അടുപ്പമുള്ള ഒപിയോയിഡ് ആണ്.[1][15]ട്രമാഡോൾ ഒരു സെറോടോണിൻ-നോർപിനെഫ്രിൻ റീഅപ്‌ടേക്ക് ഇൻഹിബിറ്റർ (SNRI) കൂടിയാണ്.[1][16]
