Very straightforward, locate the individual within the Film & Television Database and use the number from the end of the URL in the template. For example "PRATCHETT, Terry" is located at...
http://ftvdb.bfi.org.uk/sift/individual/592017
...so we take "592017" and place it into the template format to produce...

...which in turn produces...
PRATCHETT, Terry at the BFI
